Immune-effector-cell-associated neurotoxicity syndrome (ICANS). In the acute period after CAR T cell therapy, systemic CAR T therapy can lead to CRS, which may lead to break down of the BBB and globally elevated cytokines and inflammation in the CNS that manifests as ICANS, although the mechanisms of ICANS remain to be fully understood.
