Kaplan–Meier curves show the cumulative incidence of receiving a diagnosis up to 15 years after baseline, segregated into four groups according to combinations of blood and psychosocial risk quantiles (high–high, high–low, low–high and low–low). HRs are calculated using Cox-proportional hazard models, while the P value of the differences between groups is calculated using a two-sided log-rank test ( P HH < 0.001, P HL = 0.047, P LH = 0.055, P LL < 0.001).
